Medical Advocacy and Documentation

Advance Care Planning is about honoring your wishes, your personal values and your options. We follow the advice of the National Hospice and Palliative Care Organization which states, "Advance care planning is making decisions about the care you would want to receive if you become unable to speak for yourself. These are your decisions to make, regardless of what you choose for your care, and the decisions are based on your personal values, preferences, and discussions with your loved ones." 

We will meet with you to discuss your final wishes and to be sure that your documents are completed and that you are making informed choices. We will discuss forms that express your wishes for life-prolonging treatments that may provide you with quantity vs. quality of life, such as the DNR form (Do Not Resuscitate), or POLST  (Physician Order for Life Sustaining Treatment). Although all of our services are non-medical, we are also available to discuss or provide information about VSED, Medical Aid in Dying, organ donation and more.  We will assist you with organizing your advance directive, DNR, or living will, and create a binder with all your contact information. 

 

If you require a patient advocate to attend doctor appointments, take notes and assist you with and keep track of future medical plans, we are available to act in this capacity as well.
